I , agree to have eyelash extensions applied to my natural eyelashes and/or removed and retouched. By signing this agreement, I consent to the placement and/or removal of the eyelash extensions by Fancy ilash Studio.

I understand that on rare occasions there are risks associated with having artificial eyelashes and eyelash extension applied to or removed from my natural eyelashes. I further understand that in rare cases as part of the application eye irritation and some discomfort could occur. I understand that if I experience any of these conditions with my lashes I am to contact Fancy ilash Studio, as it may be beneficial to remove my eyelash extensions.

I understand and agree to the after-care instructions provided by Fancy ilash Studio for the care of my eyelash extensions. I realize and accept the consequences of failure to adhere to these instructions which may cause the eyelash extensions to fall out and/or decrease my eyelash retention. Fancy ilash Studio has discussed eyelash retention and natural lash growth with me to my understanding.

I understand and consent to having my eyes closed and covered for the duration of approximately 60-120 minutes or longer for the eyelash extension application. Times vary depending on the type of service being rendered.

I agree/ understand the following eyelash extension follow-up and maintenance instructions by marking with a check:

Avoid getting lashes wet for 24 hoursAvoid the use of steam, saunas, or sunbeds for 48 hoursAvoid perm, tint or use of eyelash curlers and mascaraAvoid oil based products around the eye areaAvoid picking, rubbing, and pulling your lashesGently wash daily with lash bath, keeping your water line area clean/non-makeup wearers 3 times a weekSchedule your fill appointments every 2-3 weeks to maintain your lash extensions.
